Direct Primary Care Memberships

Experience personalized primary care with convenient access to your provider and transparent monthly pricing. Choose the level of care and wellness benefits that best fit your needs

Tier 1
Essential Wellness

Price: 
$75 per month individual | $125 with the addition of a spouse

Includes:

✓ In-person and virtual primary care visits with no additional visit fee
✓ Visits for minor illnesses and chronic condition follow-ups
✓ Medication management and prescription refills
✓ Direct access to your provider by secure message and phone
✓ Same-day sick visits when appointments are available
✓ Annual wellness exam and one basic annual lab panel (CBC, CMP, A1c, Lipid Panel, TSH, Vitamin D), with a review of your results
✓ Access to select medications through our on-site pharmacy; medication costs are separate but very affordable
✓ No long-term contract; cancel according to the membership agreement


Tier 2
Wellness Plus

Price: 
$110 per month individual | $185 with the addition of a spouse

Includes everything in the Essential Wellness membership, plus:

✓ One wellness injection per month, when clinically appropriate, from the injections designated for this membership
✓ 5% off regularly priced, eligible IV infusions, StemWave Packages, and aesthetic services

Tier 3
Total Wellness

Price:
$175 per month individual, $325 per month with addition of a spouse

Includes everything in Essential Wellness and Wellness Plus memberships, with these upgrades:

✓ One comprehensive lab panel per membership year in place of the basic panel, with review of results (labs include CBC, CMP, A1c, Lipid Panel, Thyroid panel including T4 & T3, Vitamin D, Vitamin B, Magnesium, Iron, TIBC, Ferritin, PSA, Male/Female hormone panels)
✓ A full body skin assessment during your annual wellness exam
✓ 10% off regularly priced , eligible IV infusions, StemWave packages, and aesthetic services in place of the Tier 2 discount

Membership Details

Each enrolled spouse receives their own tier benefits, including their own annual lab panel and, for Tiers 2 & 3, their own eligible monthly injection. Membership discounts apply to eligible services at regular price and cannot be combined with any other promotions, packages, or IV membership pricing unless otherwise specified. Medications, additional labs, and outside services are billed separately unless expressly listed as included. Same-day appointments depend on availability. Direct Primary Care Membership is not health insurance.
